ADMISSIONS INFORMATION
Criteria for admission to Friends U Need Club, Inc.
• Primary diagnosis is an Intellectual Disability which may include, but is not limited to, the following: Autism, Cerebral Palsy, Down Syndrome, Fragile X Syndrome, Prader-Willi, Williams Syndrome, Acquired Brain Injury (injury occurred early in the individual's life)
•The individual is 22 years or older with mild disabilities & meets the following guidelines:
• Possesses basic self-help skills
• Able to communicate verbally & contribute to a conversation
• The individual is ambulatory
• Able to use the bathroom without assistance
• Able to function within a group without one-on-one support
• Able to focus on a task and follow directions
• Cooperative, compliant, willing, and able to engage in program activities
• The individual is not violent to themselves or others. Must not have aggressive or violent attitudes or behaviors
• The individual is not dependent on any illegal drugs or alcohol
Exclusionary Criteria – background checks required to verify criteria listed below
• History of physical aggression
• History of self-harm
• Previously or currently incarcerated
• Received in-patient treatment for psychiatric services within the last year
• History of fire setting
• Suicidal or homicidal
• Sex Offender
FUN Club members served at The FUN Club function at a high to moderate level. FUN Club members must be able to participate in activities provided in-house and field trips.
ADMISSIONS PROCESS
Assessment Interview
As space becomes available, we will contact you to schedule an interview and facility tour.
The purpose of the assessment interview is to allow our Director to determine if the applicant meets Friends U Need Club, Inc.'s basic criteria for admission. In addition, the interview helps to determine specific programs and services that best meet the needs and desires of the individual to be served.
FUN Club Facility Tour
The facility tour provides an opportunity for the applicant and his or her family to visit The FUN Club, meet our professional staff and to address any questions or concerns. The tour also provides the FUN Club team an opportunity to meet the potential FUN Club member in person and determine if our initial assessment was accurate.
Complete Application
A completed application and other required documents must be submitted and approved prior to receiving services.
Application Review
Completed applications are reviewed by members of our staff and board members. Once reviewed, the team works to determine admission status and recommendation for level of care.
Intake Process
Once an application has been approved, a start date is scheduled. On the start date, a meeting is held to gather necessary documents required for the new FUN Club member's file. In addition, a brief orientation is provided for the new FUN Club member and their family.
30-Day "Probationary Period" Meeting
A family meeting is scheduled 30 days after the start date to discuss how The FUN Club member is doing in the day program. At this time, adjustments to membership will be discussed if necessary.
